Is there any risk to my privacy in taking this survey?
No. Your answers will remain completely confidential and anonymous. You will not be asked to provide any identifying information. No individual data will be reported. Your participation in this survey is completely voluntary. No matter what your experience, or whether you choose to participate, this survey will NOT impact your eligibility for any services you receive.
